A new case report in Frontiers in Neuroscience offers a striking and hopeful observation: even in advanced Alzheimer’s disease, some functions may still be present and can become temporarily accessible under specific conditions. In this case, an 80-year-old Japanese-American woman with severe language loss, urinary incontinence, mobility dependence, and marked cognitive decline showed multidomain improvement after receiving a single 5 g dose of psilocybin-containing mushrooms.

The most remarkable changes were not subtle. Around 19 hours after administration, spontaneous autobiographical speech returned, and over the following days and weeks the patient regained urinary continence, improved mobility, dressed herself independently, made better eye contact, showed stronger emotional responsiveness, and held more lucid conversations.

Why the case matters

The encouraging message from this report is not that psilocybin “cured” Alzheimer’s disease. Rather, it suggests that late-stage neurodegeneration may leave behind residual capacities that are still available, even if they are deeply hidden.

That idea is important because advanced dementia is usually framed as a one-way decline. This case challenges that assumption and suggests that brain networks may still be modifiable, at least transiently, even after substantial loss has occurred.

A hopeful interpretation

The authors emphasize that the findings are observational and do not prove reversal of disease, but they do open a new line of thinking: perhaps some of the brain’s lost functions are not gone forever, but temporarily inaccessible.

That distinction matters. If future studies confirm this kind of response under controlled conditions, psilocybin could become part of a broader strategy for improving quality of life, communication, and autonomy in people with severe neurodegenerative disease.

What to keep in mind

This is still a single case report, so it should be interpreted carefully. Still, the breadth of the changes makes it one of the more compelling real-world observations suggesting that residual function can sometimes re-emerge in advanced Alzheimer’s disease.
